Physician Evaluation and Medical Decision Disclosure

All patients are subject to medical evaluation and approval by a licensed healthcare provider. Treatment eligibility is determined solely by the evaluating physician based on professional medical judgment, patient history, risk assessment, and applicable standards of care. Telehealth services are reduced to evaluations on history provided by patient in good faith, and it is not a direct physical  in clinic consult, if required by physician, patient will be requested a live virtual consult or clinic visit in person and will be  conducted at a facility and have a different cost, in that scenario, this program does not apply.  This telehealth service does not establish a in-person evaluation with the Medical Physician, but rather a virtual consultation. Therefore patients are advised to consult with their Primary Care Physician about this treatment before considering to start this weight loss program.

If deemed medically appropriate and approved by a licensed provider, patients voluntarily elect to proceed with treatment while fully understanding and accepting all known and unknown risks associated with compounded medications.

Patients acknowledge and agree that they assume full personal responsibility for:

• treatment participation
• medication use
• adherence to provider instructions
• reporting adverse reactions
• ongoing health monitoring
• understanding risks and limitations of treatment

FITWELLNESS.CLUB:

• does not practice medicine
• does not diagnose medical conditions
• does not prescribe medications
• does not compound medications
• does not manufacture medications
• does not dispense medications
• does not replace licensed medical care
• does not guarantee treatment outcomes or results

All prescriptions, when approved, are prescribed solely by a licensed healthcare provider and dispensed by a licensed pharmacy operating within its own legal and professional scope of practice.

• marketing
• administrative coordination
• communication support
• intake organization
• platform management

Medical decisions remain solely between the patient and licensed healthcare provider.

 By proceeding with any evaluation, consultation, intake process, telehealth communication, or treatment-related program through this platform, patients acknowledge that they:

• understand that compounded medications are not FDA-approved

• voluntarily assume all risks associated with participation
• release FITWELLNESS.CLUB from liability related to provider medical judgment, pharmacy dispensing practices, treatment outcomes, side effects, adverse reactions, or individual patient results
